Those are common issues, at least for me. The first happens whenever you try to requip/unequip/move an item and it gets cancelled by some reason or the other. The item can get lodged on the place where it is. If it's equipped you can still move it by putting another item there as you showed, but you have to relog to fix this.
The second has to do with haks. If you log out of a server and log back in without completely restarting NWN you can get this issue. |
