First off,
only melee weapons can be named by a Fighter, Duelist, Paladin or Weapon Master, and only these classes will get the chance of bonus XP and flavor message when making the killing strike with a named weapon. Other classes who use a named weapon will get neither.
On to your specific questions:
1) I am unsure about whether a named weapon can be customized at the forge. I don't think it's explicitly excluded, but it's possible it will not work due to how the customizing system actually functions. There's also a chance that it will work fine, as well as a chance that the customization will work but the name will have to be set again.
My recommendation is to customize the appearance of a weapon before giving it a name. Applying enhancements (rods and things) can be done at any time.
2) Ranged weapons cannot be named with the quill in this way (though they can be by a GM). Even if they could, they will not give a chance of bonus XP or the message because it only applies to melee weapons, and again, it will only do this for Fighters, Duelists, Paladins and Weapon Masters.
3) Per #2, bows cannot be named with the quill. My recommendation is to customize the bow before training, but it will likely be customizable after training. That's not a guarantee, just a strong suspicion.
