Location: While the effect while killed during AA has only been noticed in the Vehl Arena (where death works a little differently), the rest is in effect wherever the Duelist is.

Reproducable: With all but the stuck-while-dead bit, I can reliably say yes. With the stuck-while-dead thing, I don't know - I have only chanced upon it the once.

Edit: To Preface... I understand how low a priority this all is to fix. I just want to get this out there.
Right. So, I've gone through a lot of discussion with people about this particular class feature before, and there's a solid consensus: It's Bugged.
Just how bugged, you ask? Well, it does give attacks, it does do damage, it can only be used from a certain distance, it does a cool little jumpy thing...
However.
Firstly, when performing an Acrobatic Attack, your Dexterity score drops to 3. This is a problem, considering that A) virtually all Duelists are going to be DEX-builds with Weapon Finesse, B) DEX lends to Armor Class, and C) when your DEX is dropped in this manner, you can't do anything at all for the remainder of the round - including enter combat (that is, the character doesn't fall into stance, but just stands there getting whacked).
Screenshot. Interesting to note is the fact that, in the screenshot, while the DEX is 3, the AB is appropriate for my unarmed attack (14 BAB + 8 DEX), and my AC is appropriate as well. How this relates to my Acrobatic Attack attack bonuses, I'm not sure.

Secondly, if you are killed while performing an Acrobatic Attack (between jumping up and landing down) you get stuck in the air. You don't even heal up in the Vehl arena! And... At least in the Vehl Arena, I'm not getting that Respawn etc. screen.
Screenshot. Also, interestingly, I seem to be accruing hunger and thirst while "dead," and I can apparently equip and unequip weapons while in this state.
Relogging fixes it... Though you die again upon reentry. Haven't yet tested it outside the Arena.

Thirdly, as you can tell from the log above, there doesn't seem to be any KD associated with the ability, as listed in the LORE description. Eredel was not knocked down, and there was no save against it, either.
As to damage, I can't really say if it's doing the 1d6 per 2 levels of Duelist it should be doing. So far, counting crits, it seems to be doing the right amount of damage, but I haven't been able to test it much.
Oh! And, in the first section of log above, it apparently thinks I have come into the area... "Now entered a Full PvP area" and such. Odd.
