Starting Level

What is the norm for starting levels in this setting, do you have to start at first level?  Are there any special guidelines?  If you do start at a higher level how is this done from a setup standpoint?  Thanks.

You start at level 1, never higher.
  While multiclassing you must have a minimum of 5 levels in each class you're taking by level 20.
  You need to keep your multiclassing levels close. (Like no 1 level monk, 15 levels fighter, 4 levels monk, an even split or close like 5/7, 4/8, 6/6 would be best.) Just examples.
