Title: Items and concentration checks

If you use an item in combat I know it makes you vulnerable to an attack of opportunity but say a wand of neutralize poison or a healers hug belt, is there a chance that it will get disrupted or is the effect guaranteed becasue it comes from an item?

Items are guaranteed to work as far as I know, never had a heal potion fail me.

Only spells A) provoke attacks of opportunity and B) are subject to failure if you get hit and fail your concentration check.

Items, potions, etc. are not subject to these things, as per standard NWN rules.

Really so not even an attack of opportunity for a potion or wand?

Potions in NWN provoke attacks of opportunities as per PnP rules... at least in hardcore rules mode.

Layo runs on Normal difficulty (which is easier than real rules) so i'm not actually positive if potions etc provoke AoOs.
