Was perusing the wl requirements and I found the following:
What it takes to be eligible to become a World Leader and be approved for a (WLDQ):
The character must have worldly accomplishments. A few things that would count are: forming organizations that last and make a difference to players or other organizations, running kingdoms or being at a high level of royalty that is significant, being actively involved on quests and making a difference on quests in ways other than combat, actively assisting or fund raising in a significant way for the under privileged nations, towns, cities, or NPC's, or receiving a significant in-game title or rank. We are not basing World Leader status on combat and action, everybody does this when they are a hero - that is not what we are after when it comes to being approved for a WLDQ and in order to become a World Leader.
the item in bold is more or less impossible for a player character as I read it, since they absolve themselves of any royalty upon character creation and it would take a world leader quest to obtain that kind of power; suggest removing it.

Royal bloodlines cannot be confirmed at character creation but sometimes can be found later down the line via CDQ or even just quest situations. Kingdom running may also be accomplished through the same type of means in small or large parts. These are accurate if rare and somewhat difficult (and very time consuming!).

Has anyone even done that before? Because the closest I have ever heard a was royal advisor...

It's a leftover from when we still had plans to implement a kingdom system which never got off the ground (and never will in NWN).
The high level of royalty can probably be better replaced by high level of nobility.

Arkolio Salvorre is the closest I've heard of to running a kingdom as the husband of the daughter of the head of the Salvorre family in Katherian. One of the Ruling families of the city and kingdom.
