Dm help please

I was on earlier for Leanthar's quest when my system had issues and I was unable to reconnect. I have since fixed the issues but when I signed on I kept getting program error for central server. I then signed onto west and I was sent to bone hill from Karthy, but Twixel was killed immediately and lost xp and gp. I then was sent to Hlint from the death void.
Don't know what happened please help/advise.

Thanks for the assist on that Leanthar, I would have had more info, but after twixel was killed he was booted off and had to relog. I then ended up in hlint from the death void.
I still don't know what killed him, just a flash of lights and special effects.
