This LORE: evard's Black Tentacles needs to be looked over.

Quote
Caster Level(s): Wizard / Sorcerer 4
Innate Level: 4
School: Conjuration
Descriptor(s):
Component(s): Verbal, Somatic, Material
Range: Medium
Area of Effect / Target: Large
Duration: 1 Round / 2 Levels
Additional Counter Spells:
Save: Fortitude Special
Spell Resistance: No
Material Component: A Bit of Octopus Tentacle

A field of thick, 10 feet long rubbery tentacles rises from the ground. Each is capable of grappling a target doing 1d4+ 1/caster level (maximum 10) points of Bludgeoning damage. If successful, the target must then make a Fortitude saving throw or become paralyzed by the grappling tentacle. The tentacles are randomly spread out over the area of effect allowing no more than half of the tentacles to reach a single target in any given round. The inability of the tentacles to target small creatures makes all small creatures completely immune to the spells effects.

It says "If successful, the target must...". If what is successful? How is
it determined?

Does it really do 1d4 + 1/clvl in damage? If not, how much damage does it do?
If so, how many tentacles are spawned?

Metamagic aside, Evard's Black Tentacles spawns 1d4 + 1/caster level tentacles in the field.  Due to their distribution, no more than half of the tentacles can affect any one target in any given round, and the available tentacles are divided between targets within the AoE.

The "if successful" refers to each tentacle's attempt to grapple the target.  If successful, the tentacle will damage the target for 1d4+2 points of damage and there is a chance of paralysis.  The grapple check uses standard grapple mechanics (tentacles are considered to be "large" for the purpose of grappling).

Empower or Maximize metamagic both apply to the number of tentacles and the damage they do.
