Okay... so here I am, a 7th level wizard who had learned the use of a goof invisibility.  I've used them to scout the haven mines repeatedly.  I've been in there lots of times, have seen the opposition, and have mapped out my prey.  I have to bring back a head.  Okay, here it goes.  I go in,cast summon animal and shoot a fireball while the summons is running toward the target.  Fireball against ogres... not as good as I'd hoped.

My next spell doesn't work as I'm hit by something, and I am able to get a polymorph done, hoping that I can kill it as an Umber Hulk and then invisible myself again and get out.  Nope... I died horribly... they had critical hits and I had misses.  

So I come back to life in Hlint and I rest and meditate forever... until finally I am feeling well enough to go out once more.  I wander around the forest, bump into orcs and goblins and do them in, meet a trade and buy some gloves from him, and then I decide to check out the red light caves.

Goblins are harmless... right?

So I go in, all invisible and stealthy, and I craftily use my magic missles, lightning, and fireballs  to kill whenever the mood strikes me.  I go all the way down to the bottom and up again.  I am one level down when I am out of spells.  So it is time to rest.  I do so and I am ready to go.  So I inviso again, waltz into the room and there is a large group of goblins... I blast them with a fireball, killing all but four.  My familiar and summond dog go to work on the other four.  I figure, this is cake now... four goblins... no problem, and I think about looting bodies while the animals clean up shop.

Wait... what the... my familiar is attacking me!  Being a lowly wizard with no armor and low hit points... I was done in with three quick swipes from my own familiar before I knew what the heck happened.  

Okay... so reappear in Hlint at the bindstone... and, for some reason... I don't know why, I chose to go back to my demise site... mistake number one, because it isn't a long walk to the grave from Hlint anyway and I am also giving up the gravestone option.  So I made a big mistake with a knee-jerk reaction of "getting back into the action", but it gets worse... the site that it sends me back to was the HAVEN MINES!!!!!  Since I died in the Haven mines I've saved repeatedly and have even logged off and on about three times!

So there I was, in the Haven Mines, waaaaaaaaaay underground, with two inviso spells left and walking VERY SLOWLY because now I am severely encumbered.

I manage to get out and back to Hlint after about a year of travel, and I go back to the red light caves.  For some reason I think that perhaps since this weird bug sent me to the Haven Mines, it would give me a good bug and have a grave in the caves.  No such luck.  So here I sit, under the tree in Hlint, cursing goblins and vowing to kill every single one that I meet.

Done in by a lowly goblin... I am so envious by it's tactical ability that I hate it!

Fight in groups, depend on others, when you do, stuff like that happens alot less often

Sorry to hear that. The port to Haven mustve been a bug. But the grave vanishes if ye use the bindstone to port, cant say why it sent ye to the wrong place. :(

About yer animal. Only reason I see is maybe you hit your pet with the fireball and thus it was angered.

you were sent to haven because that is where your grave was (from the first death.)  If you die while still recovering from a death a new grave will not be created and your first grave will still be there.

If you had fully recovered and rested (death effects were removed) before you died to the goblins, then indeed it is a bug that you were sent to the first death site, and you should post a summarized version of this thread detailing the exact process (death, recover by waiting, rest to remove death effects, death, port to original grave when using the option on the bindstone) under bug reports.
