Only one of them respawns, the one closest to the door, the one behind the goblin warlord never respawns, except on a server reset.

Additional Info: I've been to collect gems there often in the last few months and never seen a problem with the spawns respawning.  Maybe I'm just unlucky, heh.

Is it possible that the spawns are acting odd because West hasn't been reset in about five days?

It's also possible for a spawn's respawn (which is a DelayAction) to fail to fire due to server overload at the crucial moment - which will cause the spawn to vanish until server reset.

I'm inclined to believe that this is what has happened, since I've not seen anything wrong with that particular spawn before, and there's nothing non-standard about it.

All the CNR respawns using the same system, so it's extremely unlikely this is a module bug and is, as has been said, a result of lag and/or server up-time.

Edit:  Further, there have been no module updates in months, so the likelihood that something has changed in the last couple of weeks is extremely slim. (read: not possible)

I understand that.  What I'm saying is that it is entirely possible for the respawn to fail in a couple different ways.

The first is a simple miss-fire of the respawn script due to high lag or server load.  It's infrequent, but it happens.

The second is if too much Area-of-Effect firepower is directed in the vicinity of the CNR and most specifically the hidden object that respawns the CNR, it can, in fact, become destroyed and not respawn that way.

There's an equal chance of either in this case, I believe.  CNR positioned behind a boss creature could very well be subjected to such conditions as I described as the second possibility.

Neither case is a "bug" as such and will in all likelihood act correctly when the module is reset again.

Does anyone ever hear the voice of Spock or maybe even Data (From Star Trek the Next Generation) when Dorg speaks?  *smiles and hopes that Dorg takes that as a compliment*

I just verified this in-game.  It respawns just fine.  No bug.  The respawn object likely got destroyed due to overactive AoE spells, or it's just a victim of lag.
