What is the damage on a Headbutt?  Does one have to make a save of any kind to see if they knock themselves out?

A monk or an unarmed fighter should allways use the forehead & teeth & elbows & knees as I was tought by my marshal arts master in my 8th year back in 1972

In this situation, using teeth against a metal armor... well you can guess how it would end like. ;)
